We have an exciting opportunity available for a Whānau Ora Navigator to join our friendly and collaborative team. In this role you will be responsible for building effective relationships with whānau and working alongside them to support their goals of self-management and responsibility for their own health, economic, cultural and social development wellbeing. You will achieve this through the coordination and facilitation of appropriate activities, services, resources and opportunities as identified with whānau during the planning process.
